First Visit

Brief Paperwork

Upon entering the office, we have some brief forms for you to fill out. The questionnaires provide us with general information about yourself and your condition. To fill out the new patient form prior to your visit.

Bring a list of your current medications, health insurance card, any recent x-ray/MRI reports pertaining to your visit or make sure that your referring physician has sent all of your pertinent information to us, including a referral if necessary. Any co-payment required by your health insurance will be expected at each visit.

If you are a Worker's Compensation or Auto Accident related case, in addition to the above, please bring the following information:

For Worker's Compensation

  • Occupation when injured
  • Date of the accident
  • Employer's name, address and telephone number
  • Employer's insurance carrier name, address, telephone number and policy/claim number (this is not the same as your health insurance carrier)
  • Name and telephone number of the claim adjuster
  • Description of where and how the accident occurred

For Auto Accident

  • Police report of the accident
  • Auto insurance information (yours and the third party's) including your declarations page
  • Claim number and adjuster's name and contact information (if you have one)
  • Attorney information (if you have one)

Consultation

Once the questionnaire has been completed, you will have a consultation with the doctor to discuss your health-related problems and concerns, as well as potential treatment options.


History and Examination

In order to determine what your condition is, the doctor will ask you various questions related to your symptoms. Clear communication is very important. Make a list of your health concerns as they relate to chiropractic. If necessary, you can write down information given by the physician so that you are sure you clearly understand your condition. Next, a number of specialized tests will be performed to determine which tissues/joints are affected and to what extent.


X-ray Studies

Your specific condition may require us to take x-rays to either rule out more serious conditions or assist us in developing the most effective treatment plan for you.


Same Day Treatment

Once finished with the examination and all appropriate studies, patients will generally be provided with their first treatment during this same visit. This may include spinal adjustments, physical modalities, and/or soft tissue therapies.


Home Instructions

Prior to leaving, patients will be given instructions on certain activities or procedures to be conducted at home. This may include ice or heat application instructions, avoidance of certain activities or positions, as well as home exercises and/or stretches.


Schedule Your Next Appointment

You will be given a convenient appointment time for your follow-up visit. Generally speaking, patients are seen within 1-2 days and are provided with a complete review of findings. Treatment options for your condition will also be discussed during this time.
